You may need the philips screwdriver again to free the door from the track.
Out of square sliding glass door.
Every sliding glass door has adjustment screws that hold the rollers of the door in place.
Once those two steps are taken you should be able to carefully remove the door from the track.
Just pull the hinge pins out of place tapping them from beneath with a screwdriver and mallet if necessary.
Then remove the door stop at the top of the frame which should be as simple as unscrewing it.
Then tilt it slightly with help and remove it from the upper track of the door frame as well.
Then gently lay it flat on a foam sheet.
While you might be able to jack up the house with the door frame in place you d need to go slowly since it took many years to settle to keep from breaking or damaging the wood frame or any plaster on the walls.

For the trim wedge a flat pry bar underneath the edge on one end of a section and gently pull the casing away from the wall.
The best solution is to square the frame back up by jacking up the low side from under the house then your door will work fine.
